Visitors heading to India from international destinations are now required to complete a digital self-declaration form before arrival, in response to the ongoing Ebola disease outbreak.

The Air Suvidha 2.0 Passenger Health Self-Declaration Form should be completed within 24 hours of the start of passenger travel to India, indicating a visitor’s travel history over the preceding three weeks, including any visits to Ebola-affected countries.

Upon submission, passengers will receive an acknowledgement via email, to be presented to immigration officials upon arrival in India.

The mandatory requirement applies to all international passengers arriving in India, including Indian passport holders returning from overseas.

Officials have noted that this health self-declaration form is separate from the electronic arrival form required upon entry into India, and from any Indian visa application process.
